Insurance and Patient Co-payments
Dental insurance can play a significant role in your treatment costs, so we recommend completing your new patient paperwork with your insurance details in advance. This lets us verify your benefits and prepare an estimate of any fees or co-payments before your visit. Our team is always happy to work with your provider and answer questions about your policy.
Please note that you are ultimately responsible for any fees and co-payments your plan does not cover. We're glad to help you file your dental or medical claims, and we ask that payment be made according to our financial policy when it's due.

Appointments and Consultations
At your visit, Dr. Hays will perform a comprehensive exam and review your dental and medical history. Most routine appointments include a checkup and professional cleaning, and if we notice any signs of decay, damage, or gum disease, we'll talk through your options together. Once we agree on a treatment plan, our front office team will provide a clear breakdown of fees, along with an estimate of your out-of-pocket costs based on your insurance.
